Corrections ledger

Corrections to the record arrive as signed, cited counter-claims over Nostr, pass through the same two-person editorial gate as any record change, and land here in the open. A correction that can’t name a source is a coverage finding, not a claim — so every entry below cites one. Nothing is auto-applied: the ledger records what was accepted; a human reviewer (never the record’s own editor) authors the change.
